In an effort to ensure each passport applicant has ample time to have their application reviewed and processed, we ask you to be on time for your appointment, have your paperwork ready and be prepared to write multiple checks.

If you are more than 5 minutes late to your appointment, don’t have your paperwork ready or are not prepared to write checks, you may need to re-schedule to another date.  This is to ensure that each scheduled appointment will have the designated and necessary time to process the applications.

The City of Maple Valley is an authorized Passport Acceptance Facility, allowing us to assist with applications for U.S. Passports. Passport Applications are accepted at Maple Valley City Hall, located at 22017 SE Wax Road, Suite 200, on Tuesdays and Wednesdays from 9:30 a.m. – 3:00 p.m., by appointment only. The last appointment will be scheduled at 2:30 p.m. to ensure timely processing. Please Be Aware: The actual decision to issue or deny a passport is made only by a Passport Services Agency, not the City of Maple Valley. To check on the status of a submitted passport application, please call the National Passport Information Center at 1-877-487-2778 (TDD: 1-888-874-7793) or visit the State Department's website. 

APPLY NOW TO AVOID DELAYS:  We are expecting an increase in passport applications through 2018.  Submit your applications well ahead of your planned travel dates.  In 2007 it was a new requirement (Western Hemisphere Travel Initiative/WHTI) to have a passport when traveling to Canada, Mexico, Caribbean, and Bermuda.  There was a surge of passports issued in 2007 and 2008, and those passports are now 10 years old and are expiring.

What Do You Need To Apply For A Passport?


(Form DS-11) is used for First-Time Applicants and Applicants not eligible for Renewal by Mail (DS-82)

See below (in the 2nd second section) the requirements to be able to Renew by Mail (DS-82)

Please Note: all applicants, including minors, must appear in person when the applicant applying is applying for the first time or when an applicant not eligible for Renewal by Mail.

  • Complete Form DS-11, Application for a U.S. Passport (BEFORE ARRIVING FOR APPOINTMENT)
    Applications are available at City Hall or click here to download an application from the State Department's website.
    • Do not sign the application until instructed to do so by the Passport Acceptance Agent
    • The applicant’s Social Security Number must be provided on the form (question #5)
    • NO double-sided coping, when printing the form at home.
    • DO NOT use white out on the application.  If you make a mistake, put a single line through the mistake, and write above, below, or beside the mistake.
    • When printing the application, sometimes the printer settings will shrink the page.  If this happens, the application will NOT be acceptable.  Make sure the application takes up the whole page.  This is because the application will be scanned, and the information needs to be in its proper location for the scan to work properly.

Failure to provide information requested on the form, including the SS number, may result in significant processing delays and/or denial of the application.

  • Submit Evidence of U.S. Citizenship and Provide a Photocopy (of front and back/can not be double-sided)
    Provide one of the following:
    • Certified birth certificate* issued by State, County, or City (hospital certificates are not acceptable)
    • Previously issued, undamaged, U.S. Passport
    • Naturalization Certificate
    • Certificate of Citizenship
    • Report of Birth Abroad

Additional documentation may be requested to establish U.S. Citizenship.

* A certified birth certificate has a registrar's raised, embossed, impressed or multicolored seal, registrar's signature, and the date the certificate was filed with the registrar's office, which must be within 1 year of your birth. Please note, some short (abstract) versions of birth certificates may not be acceptable for passport purposes. Beginning April 1, 2011, all birth certificates must also include the full names of the applicant's parent(s). Notarized copies or photocopies of documents are not acceptable. Original documents must be submitted with the application and will be returned when the passport has been completed.

  • Present Identification and Provide a Photocopy
    Present one of the following and provide a photocopy of the front and back (can not be double-sided) of the identification that you will present when you apply:
    • Valid Driver’s License
    • Naturalization Certificate
    • Current Government ID
    • Current Military ID

Additional documentation may be requested to establish identity.

  • Provide One Passport Photo
    Photo must be 2” X 2” in size, taken within the last 6 months, clear, front view with no hats, no glasses are allowed in photo, taken in normal street attire, and printed with a white or off-white background.  Click here for photo requirements.

  • Pay the Applicable Fees
    Two separate payments are required per application. For multiple applications, a separate check or money order is required for each application. Fee schedule listed below.  The payment that goes with each application must be check or money order.  The payment to the City can be cash, check, money order, or credit card (VISA or MasterCard only).

Special Requirements for Minor

Minor under age 16

The minor and both parents or legal guardian(s) must appear and:

  • Submit evidence of the child’s U.S. citizenship
  • Submit evidence of the child’s relationship to the parent(s)/guardian(s) (ex. certified birth certificate including parent(s) names; a previous U.S. passport is not acceptable as evidence of relationship) **
  • Parent(s)/guardians(s) must present identification and provide a photocopy (front and back) of identification

If both parents or legal guardian(s) are not able to be present, a notarized Statement of Consent
(Form DS-3053) must be submitted along with a photocopy (front and back) of the non-applying parent’s/guardian’s identification; or other supporting documentation must be provided. Click here for more information.

** If the name of a parent/guardian has changed since the original documents were issued, evidence of a legal name change must also be submitted (ex. certified marriage certificate).

Minor ages 16 – 17
For security reasons, parental consent may be requested. If your child does not have identification of his/her own, you need to accompany your child and present identification as well as provide a photocopy (front and back) of your identification.

Passport Fee:

Passport Renewals By Mail (Form DS-82)

Form DS-82 can be used to apply for a passport book or a passport card by mail if you meet the following requirements:

You may qualify to renew by mail only if YOUR MOST RECENT U.S. PASSPORT:

  2. Was issued when you were age 16 or older; and

If you answered "yes" to ALL of the 4 questions, you may process your renewal by mail. You must be sure to sign and date this form before mailing. If you can't answer yes to all of these 4 questions, you will need to apply in-person as a first-time applicant (using the DS-11).

How Long Does It Take?

  • Routine Service: Processing time for new, renewal, and mail-in passport applications is approximately 6 weeks, but may take longer during peak travel periods.

  • Expedite Service: If you need your passport before the normal processing time, you may request expedited service. It could take about three weeks to receive an expedited passport. Cost for expedited service is an additional $60.00 per application and overnight delivery service is strongly suggested. Please inquire for postal rates.

  • Seattle Passport Agency: Customers can also apply in person at the Seattle Passport Agency. All customers must have an appointment and pay the required expedite fee of $60.00. The Agency does not accept routine passport applications.

    • The agency offers five day service for first-time applicants, passport renewals, data changes, additional pages, and applications for the passport card.

    • Will-call service is offered for life/death emergencies and immediate travel abroad (leaving within 5 business days) provided the applicant shows proof of travel/emergency.

Customers can schedule an appointment 24 hours a day, 7 days a week, using the automated appointment system at 1-877-487-2778. Business hours are from 8:00 a.m. to 3:00 p.m., Monday through Friday, excluding federal holidays. The Seattle Passport Agency is located at 300 Fifth Avenue, Suite 600, Seattle, WA 98104.
